For Patients
About Your Visit
Dr. Stephen Stanley looks forward to meeting you! On your first visit please arrive 15 minutes prior to your appointment time and have the provided you with forms that need to be completed. If your insurance requires a referral please bring it with you to your appointment.
Also have the following items with you:
- Valid Drivers License or State ID
- Insurance card(s)
- List of medications you take on a regular basis, including over-the-counter
- medications and vitamins.
- List of surgeries or medical procedures you have had
- Payment for your co-pay or any non-covered services. For your convenience,
we take cash, check, VISA, MasterCard, and Discover.
Patient Forms
Please download, print and complete prior to your appointment.
All your medical records are considered confidential information. We will release your records, or some part of them, only with your written authorization. No matter what the circumstances, I will not release these records unless I have written notification that you wish for me to do so. Please read our complete Privacy Statement above.

Prescription Refills
If you are calling for a prescription refill then please call your pharmacy. If you desire a new prescription or to change your pharmacy, then please call me during office hours when I will have access to your chart to ensure the best course of treatment. These changes will only be made during office hours. Please have the phone number for your pharmacy available at the time of your call.

Insurance
I participate in many different PPO and HMO plans and accept traditional indemnity insurance. If you are not sure about our provider status with your insurance company, please check with your insurance company or with my business office during normal office hours.
